PHC has no participating physical therapy providers. Refer to the HMSA HMO Referral Panel for these services.

PHC members who see a Physical Therapist provider who is in the HMO Referral Panel DO NOT NEED to obtain an Out-of-Network Referral approval.

DME
PHC has no participating Durable Medical Equipment (DME) providers. Refer to the HMSA HMO Referral Panel for these services.

PHC members who see a DME provider who is in the HMO Referral Panel DO NOT NEED to obtain an Out-of-Network Referral approval.

Optometry
PHC has no participating optometrists. Participating PMAG ophthalmologists should provide this service.
